Thank you for completing the online New Student Counseling Workshop. Please complete this intake form so that a new student counselor can assist you in creating your first educational plan. Accurate responses will help us create an individualized plan that matches your goals. There are five sections on this form.
You will see a confirmation on your screen when you have completed all of the questions. This online form should be filled out in one sitting and can take between 15-20 minutes.
It is important that you begin to check your PCC Email Address. You will be notified via email when your education plan is ready to be seen in your LancerPoint Account. Student can anticipate receiving their educational plan within 14-15 business days. If you are unable to complete this form online for any reason, please contact us at pathways@pasadena.edu.
If you have already submitted this form, please do not submit another form as that will move you to the end of the queue for educational plan requests. We appreciate your patience as counselors get through all submissions.
Important term dates:
Winter 2025 Term (Intersession): January 6, 2025 - February 13, 2025 (6 weeks)
Spring 2025 Term (Semester): February 18, 2025 - June 16, 2025 (16 weeks)
Before you start this form, you will need:

- An unofficial copy of any outside transcripts of completed college units or exam scores (community college, university, IB or AP Scores)
